You can't sleep. You feel in the dumps. You can't concentrate and feel sluggish. And yet, a health check comes back with the result, "No problems."
　眠れない。気持ちが沈む。集中力もなく体がだるい。でも、健康診断の結果は、「どこも異常なし」。

If you are someone with an interest in mental health, this alone might be enough to make you suspect depression and go visit a mental health clinic. The doctor's diagnosis is, as you suspect, depression. If you are someone knowledgeable about this area, you might think, "Now I'll be prescribed antidepressants and told to rest at home for a few months. No problem, I've already gotten permission from the boss to take time off."
　あなたがメンタルヘルスに関心のある人なら、これだけで「あ、これがうつ病かな？」と思い、メンタルクリニックを訪れるかもしれない。医師の診断は予想通り「うつ病ですね」というもの。さらに、この分野にくわしい人なら、「きっと抗うつ薬が処方されて数カ月の自宅療養を勧められるな。もう上司にも休みの許可は取ってあるから大丈夫だ」などと思うことだろう。

However, the doctor instead says something unexpected: "Alright, why don't you come into this room for some magnetic stimulation treatment. There aren't any side effects. You just wear a device like a pair of headphones for around 30 minutes. You only need to come in for it around three times a week. For people for whom the treatment works, the effect appears quickly, so you won't need to take time off from work, either." In the room you enter into are chairs like those at a dentist. "Take a seat," says a smiling technician holding one of the devices.
　しかし、ここで医師は思いがけないことを言う。「では、こちらの部屋に来て、磁気刺激治療をしましょう。あ、痛みも副作用もまったくないですよ。ヘッドホンのような装置をつけて約３０分。週に３回くらい通ってもらえば結構です。効果がある人は、すぐに効きますから会社も休まなくて大丈夫です」。そして、通された部屋には歯科の治療台のようなチェアが並び、女性技師さんが頭につける装置を手に、「さあ、おかけください」とにっこり……。

It may sound like science fiction, but this scene could become reality in the near future. Currently, this so-called "transcranial magnetic stimulation" (TMS) that treats depression symptoms by improving blood flow is under the spotlight in the United States. It is also offered at some Japanese institutions, although it is not covered by insurance, and recently some Japanese companies have begun making these devices.
　なんだかＳＦ映画のような話だが、これが近い将来、現実になるかもしれない。
　今、米国では頭の表面に磁気刺激を与え、それが脳に伝わって血流を改善させることでうつ症状を改善させるという新しい治療、「経頭蓋（ずがい）磁気刺激」（ＴＭＳ）に熱い注目が集まっている。日本でも一部の医療機関が健康保険の利かない自由診療で実施しているのだが、このほど日本のメーカーも、この装置の市場に参入するなどして、さらに広まる勢いを見せている。

Compared to the past, antidepressants nowadays have far less problems with side effects, but still many people are averse to using them. For such people, magnetic stimulation treatment must be very attractive.
　うつ病の治療では抗うつ薬を使うことが多く、今は昔に比べて副作用などのデメリットは格段に小さくなったのだが、それでも「薬には頼りたくない」「抵抗がある」とためらう人は少なくない。そういう人たちにとっては、「磁気による刺激だけで効果がある」というのは、大変な魅力だろう。

However, I have a small concern. Should this TMS become the standard treatment, will the need for conversation between patient and doctor end? After a doctor makes a diagnosis, will the rest be left to the machines? I think of patients who, after getting better said that although their depression was difficult, they liked being able to relax and talk to the nurses and doctors at the hospital. Or is that only me romanticizing about what is becoming the mental care of the past?
　「でも」と、私はちょっぴり心配でもある。このＴＭＳがうつ病の治療のスタンダードになったら、患者さんと医師との対話は必要なくなるのだろうか。診断をつけたら、後は治療は機械にお任せ。本当にこれでいいのだろうか。「病気になったのはつらい体験だったけれど、この病院に通って看護師さんや先生とゆっくり話せたのはよかった」と、治った後に語ってくれた人の笑顔が浮かぶが、それは今や“旧世代の精神科医”になりつつある、私のただの感傷にしか過ぎないのだろうか。
