The Ministry of Education, Culture, Sports, Science and Technology is reportedly considering establishing "international medical schools" where students will be trained to become leaders in the global medical field. The move is part of the human resource development for National Strategic Special Zones -- model cities with few regulations to invite new businesses that the government plans to set up -- where people will have access to the world's top medical services.
　文部科学省は、世界の医療分野をリードする人材を専門に育てる「国際医学部」が必要と判断して、医学部新設の検討を始めたそうだ。政府は世界最高水準の医療を提供できる国家戦略特区の設置を計画しており、そのための人材育成を目的としているとのことだ。

The level of medical care in Japan is one of the best in the world. I wonder how advanced medical services will be at international medical schools where the world's best professors and students gather to learn. The official language at the institutions, of course, will be English and the staff will fly around the globe, having a lecture in Paris one day and surgery in London in the same week. Under the plan, celebrities from across the world will come to Japan to receive the world's best medical checkups and treatment, supporting the Japanese economy.
　日本の医療水準は現在でも世界有数を誇っているが、世界で活躍する教授らと優秀な学生が集まる国際医学部は、どれほどの高レベルになるのだろうか。もちろん公用語は英語で、スタッフは「明日はパリで講演、週末はロンドンで手術」と、世界を飛び回るのだろう。世界からセレブが超一流の健康診断や治療を求めてやって来て日本経済の後押しもしてくれるに違いない。

This all sounds very glamorous, but I can't be so happy about the plan when I look at the reality. Many hospitals and clinics in Japan, especially in rural areas, are suffering from doctor shortages. Local public hospitals that are supposed to serve as the center of medical services in the area don't have enough full-time doctors and are having to close down departments for outpatients. Medical institutions in urban cities also lack doctors in fields such as child psychiatry.
　夢のような華やかさだが、ふと現実に引き戻されると手放しで喜ぶわけにもいかない、という気分になる。日本では今、あちこちの医療現場、特に地方では深刻な医師不足にあえいでいる。地域医療の中核を担うはずの公立病院からも常勤医がいない、外来を閉鎖せざるをえない、という声が聞こえてくる。また、都市の病院であっても、小児精神科などまだまだ人材不足に悩む科も少なくない。

I am currently working at a private clinic in Tokyo as a part-time doctor. Every time I check my email at work, I see a number of job offers sent from career information service providers for doctors. I have no intention of leaving the clinic, but emails consisting of phrases like, "Job offer with plenty of days off and high salary," or "You can work in central Tokyo," sound attractive even to me. If I was working at a busy hospital in a rural area, I would have chosen the high paying job in Tokyo over the demanding work in a short-staffed hospital. I don't have the heart to blame doctors who leave hospitals in the countryside to work at clinics in cities.
　私は現在、東京で民間の診療所に非常勤で勤務しているが、パソコンのメールボックスを開くと、毎日のように医師の転職情報サービスからメールが届いている。私自身、転職などまったく考えていないのだが、「年収も休日もたっぷり」「都心で働きませんか」といった魅力的な誘い文句を日々見せられるとなんとなく気になってくる。もし今、地方で多忙な病院に勤めていたら、きっと誘いに乗ってしまったのではないかと思う。そう考えると、激務を離れて都市のクリニックに移る医者を責める気にはとてもなれないのだ。

I imagine ambitious doctors and medical students who aspire to challenge themselves through the world's top level medicine would want to work or study at the proposed international medical schools. Their ambition should not be restricted, but it would create vacant positions in regular regional medical institutions.
　文科省が考えている「国際医学部」はどうなのだろう。世界水準で腕を試したい、という野心を持つ医者や学生は、そこでの勤務や勉強を希望するのではないだろうか。向上心は否定されるべきではないが、その分、一般の地域医療に“空席”が生じることになる。

It would be ideal if both the world's top medicine and community hospitals could advance in their own ways, but we all know that is unrealistic. So, what we need to do here is to prioritize between the development of the "best medical services that only a selected few can benefit from" and "general medical services that offer care for everyone."
　もちろん、理想は最高水準の医療も地域医療も、ともに充実することだ。とはいえ、それが現実的でないことは誰の目にも明らか。となると、少数のセレブが恩恵を受ける「世界最高」と無数の人のための「地域一般」、どちらのレべルを優先すべきか、ということになるのではないか。
